Job summary

The Mishkon Division of The Jewish Board is seeking a Habilitation Support Professional in New York to assist individuals with intellectual and developmental disabilities in home and day habilitation settings. You will follow ISP/Day Hab plans to promote independence and person-centered decision making.

Responsibilities include ensuring safety, supporting daily activities, documenting progress, and participating in treatment planning with the interdisciplinary team.

Qualifications

• High School diploma or 1 year experience with IDD population.
• AMAP certification is required during probation for Day Habilitation.
• Experience with individuals with Intellectual/Developmental Disabilities preferred.

Responsibilities

• Ensure client safety and promote independence in home or day habilitation settings.
• Assist with personal hygiene and daily activities per ISP/Day Habilitation Plan.
• Document observations and progress in the electronic health record.
• Assist treatment planning meetings and implement valued outcomes.
