You will work as part of an inter-disciplinary team, provide transportation when needed, and document progress while adhering to agency policies and safety protocols.

Qualifications

• High school diploma or equivalent preferred.
• Must have a valid NYS driver's license and maintain driver status.
• Ability to understand and communicate written and oral directives/information.

Responsibilities

• Provide ongoing guidance to individuals to support independence in daily routines.
• Assist with self-care, health, safety, and household tasks as needed.
• Support community integration and social participation.
• Assist with transportation to appointments and activities.
• Document activities and follow agency procedures.
