HASC Center in Brooklyn, NY is seeking Direct Support staff to assist individuals with intellectual and developmental disabilities in daily living, community participation, and skill development. You will support ADLs, document observations, and advocate for person-centered plans in a warm, respectful environment.

Responsibilities

• Be familiar with IPOP, Life Plan, Staff Action Plan, Behavior Plan to understand care management.
• Assist individuals with ADL and independent living skills per plan documents.
• Promote individualized services and adhere to plans.
• Document outcomes using a computer program.
• Move and assist with physical tasks as needed.
• Accompany individuals on community outings.
• Maintain safety and follow policies and procedures.
• Attend trainings and maintain certifications and driving authorization.
