Job summary

Heart Havens seeks a caring Direct Support Professional to empower adults with developmental disabilities to live full, vibrant lives. You will support daily activities, plan trips, and help residents pursue their passions in a compassionate team environment.

We value accountability, integrity, respect, and empathy. Candidates should have the ability to transport residents, meet health protocols, and complete detailed ISP notes and reports.

Qualifications

• High school diploma or GED preferred.
• 6 months’ direct support experience required.
• 1 year direct support experience in a DD residential group home preferred, or 2 years in DD residential with consistent job history.

Responsibilities

• Provides support to residents per ISP and Residential Plan for Supports.
• Supervises residents and never leaves them alone in home or community.
• Supports residents with Activities of Daily Living to needed level.
• Encourages residents to engage with others and participate in meaningful activities.
• Administers medications according to procedures and the 6 Rights of Medication Administration.
• Completes accurate daily documentation and reports health updates; maintains confidentiality.
