Responsibilities

• Develop and implement individualized support plans in collaboration with clients, families, and interdisciplinary teams.
• Facilitate community engagement and social activities to promote inclusion and skill development.
• Monitor and document client progress, behaviors, and any changes in health or well‑being, ensuring accurate and timely reporting.
• Advocate for client rights and needs while maintaining a safe and respectful environment.
