Job summary

Centerstone in Indianapolis, IN seeks a Community Support Specialist to help individuals adapt to community living through personalized plans and evidence-based mental health services. You will collaborate with clients, families, and local providers to implement plans, coordinate care, and maintain accurate records while promoting independence.

Qualifications

• Bachelor’s degree in psychology, social work, or related field.
• Two years of related work experience or four-year combined education/experience.
• Experience delivering one-on-one or group services with rehab/habilitation focus.
• Ability to coordinate care with families, schools, and other providers.

Responsibilities

• Provide community-based mental health services including assessment and treatment planning.
• Collaborate with clients, families, and stakeholders to implement plans.
• Use evidence-based practices such as Motivational Interviewing, CBT, DBT.
• Coordinate care with primary care, psychiatrists, schools, and community orgs.
• Maintain records per policies and participate in training and QA initiatives.
