disAbility Cornwall & Isles of Scilly is seeking a Personalised Support Specialist to help disabled people, those with long-term conditions and unpaid carers live more independently at home and in the community across Cornwall and the Isles of Scilly. You will work with a range of partners to deliver practical, strengths-based support.

Responsibilities

• Deliver personalised support in homes and communities, aligned with individual plans.
• Support people after hospital discharge and prevent avoidable admissions.
• Conduct holistic assessments with clients using a strengths-based approach.
• Provide practical support such as shopping and life administration.
• Build positive, respectful relationships with clients.
• Collaborate with internal teams to ensure effective delivery of support.
• Liaise with health/social care professionals and community partners.
• Help people access local services and opportunities beyond direct support.
• Maintain clear, accurate case records and uphold safeguarding.
