Job summary

Support Solutions provides hands-on care for individuals with disabilities in the Telford area, helping them lead self-directed lives and build daily living skills. The role emphasizes respectful, person-centered support and empowerment within the service framework.

The position involves assisting with daily activities, transportation, meal prep, and documentation while ensuring confidentiality and safety in all interactions.

Qualifications

• Must be 18+ years and able to read/write effectively.
• ECF staff must have a high school diploma or GED.
• Valid driver’s license with acceptable driving record.
• Pass state/local background checks and required screenings.
• Must meet all training requirements and keep training current.

Responsibilities

• Supports the person supported with personal care and daily routines per ISP.
• Helps the person supported pursue community activities and goals.
• Assists with transportation to events, job sites and medical appointments.
• Maintains electronic shift notes and documents progress toward outcomes.
• Provides meals and assists with household tasks as scheduled.
