Challenge Unlimited, Inc. in Swansea, IL seeks a Community Day Services Manager/QIDP to provide case management and person-centered services for individuals with disabilities.

The role develops service plans, monitors progress, ensures regulatory compliance, and supports clients toward greater independence in the community. The position requires CPR/First Aid, CPI, QIDP and Direct Support training (provided by the company) within the first 4 months and ongoing.

Qualifications

• Requires a Bachelor's degree in a human services-related field or RN licensure.
• Minimum 1+ years of experience working with people with developmental disabilities.
• CPR, First Aid, CPI, QIDP and Direct Support Person training provided by the company must be completed within 4 months.

Responsabilités

• Provide case management and person-centered services for individuals with disabilities.
• Develop and implement individualized service plans and monitor progress toward goals.
• Ensure regulatory compliance and assist with transportation and community integration activities.
• Supervise facility-based and community-based services and coordinate client activities.
• Assist with hiring, training, and development of team members.
