Job summary

Catholic Charities in Toledo is seeking a Case Manager to help stabilize families in crisis by providing resources, education, and support in a safe environment. You will develop and implement an individual service plan with clients and may pursue Community Health Worker certification.

The role involves coordinating services, advocating for clients, maintaining case files, and entering data for reporting. Travel within the Diocese of Toledo and collaboration with partner agencies are expected.

Qualifications

• Bachelor's degree in Social Work, Psychology, or Sociology with 1-3 years in social services or case management.
• Proficiency with Microsoft Excel and Word, and knowledge of local community resources.
• Maintain a valid driver's license and ability to travel within the Diocese of Toledo.
• Background check (BCI/FBI) required prior to employment and every 5 years.

Responsibilities

• Conduct initial assessments and coordinate services, empowering clients to overcome barriers.
• Provide ongoing help navigating services, offering referrals, and advocating for client needs.
• Help clients set personal goals such as budgeting, job search, education, and life skills.
• Connect clients with housing, healthcare, mental health services, employment programs, and benefits.
• Maintain accurate case files and report outcomes to supervisors.
• Enter client data in tracking databases and support monthly statistics and reporting.
• Assist the Program Coordinator with statistics and activity reports.
• Provide life-skills instruction, mentorship, and community advocacy; conduct periodic home visits.
• Perform other duties as assigned by the supervisor.
