Job summary

Sunrise Community in New Milford, CT is seeking Direct Support professionals to assist individuals with intellectual and developmental disabilities in daily life within the community.

You will provide support, supervision and programs per plans, document progress, and ensure health and safety while promoting independence. This role requires a background check, a valid driver's license with endorsements, and basic computer skills.

Qualifications

• Must successfully complete pre-employment Background and Drug Screenings.
• Must possess a valid Driver's License with endorsements and have 3 years driving experience.
• Must possess basic computer skills.
• High School Diploma or equivalent if required by work location.
• Minimum one year of experience in human services if required by work location.

Responsibilities

• Identifies and reports any suspected abuse or rights violations.
• Provides support, assistance and supervision to meet individuals' needs and goals per plans.
• Ensures training, behavior and support programs are implemented and documented per regulatory requirements.
• Ensures health, safety and well-being of individuals are protected at all times.
• Provides a positive environment that promotes independence and choice.
• Follows positioning schedules and uses equipment per plan.
• Provides necessary level of assistance for diet, texture and equipment.
• Keeps homes, outside areas and vehicles well maintained and organized.
• Performs all other duties as assigned.
