Job summary

Sunrise Community in Connecticut seeks a Direct Support professional to assist individuals with intellectual and developmental disabilities in living valued lives in the community.

Responsibilities include providing support per individual plans, ensuring safety, documenting progress, and promoting independence. Requires a HS diploma, a valid driver's license with endorsements, and at least one year in human services if location requires it.

Qualifications

• Must possess basic computer skills.
• Must possess a valid Driver's License with endorsements and 3+ years driving experience.
• High School Diploma or equivalent as required by work location.

Responsibilities

• Identifies and reports suspected abuse or violations of an individual's rights.
• Provides support and supervision to meet individuals' plans and goals.
• Ensures training, behavior and support programs are implemented and documented per regulations.
• Maintains health, safety and well-being of individuals at all times.
• Promotes independence and choice in a positive environment.
• Follows positioning schedules and uses equipment per plan.
