Job summary

Opportunity Partners is seeking an Employment Specialist II to coordinate services and support individuals with disabilities toward independent community employment in the East Metro area. You will help clients grow skills and gain independence while working with a collaborative team.

The role operates Monday through Friday, 8:00 AM to 4:30 PM, with an hourly wage of $19.00 to $20.25 based on experience. This position emphasizes direct service, case management, and outreach to employers to

Qualifications

• A designated coordinator may have varying combinations of degrees and years of experience as listed above.
• All listed qualifications relate to direct care services for persons with disabilities or older adults.
• Supervision by staff meeting the coordinator qualifications is required for certain roles.

Responsibilities

• Assist individuals with securing community-based employment according to their preferences and the Employment Plan.
• Assess and document the need for support and provide on-site training as needed.
• Provide case management to initiate, provide, evaluate and communicate services to internal teams and IDT members.
• Develop and coordinate a support plan highlighting employment strengths for independent work.
• Market services to employers and referral sources to obtain placements and related services.
• Complete intake and annual paperwork to meet billing requirements.
• Help people develop and enhance work skills in real-life settings.
• Support a meaningful job experience that improves lives and social skills.

Driver's license

Education

Baccalaureate degree in a field related to human services, education, or health

Associate degree in a field related to human services, education, or health

Diploma in a field related to human services, education, or health

Minimum 50 hours of education and training in human services, education, or health and four years full-time work experience
