Reports To: Director of Exceptional Children Services

Supervises: N/A

Job Goal: Implements the Community Work Transition Program (CWTP) in coordination with HDI and OVR; coordinates community sites used for CWTP; serves as a team member managing the program. Provides work-based learning experiences, post-secondary counseling, workplace readiness training, and self-advocacy instruction. Completes vocational assessments, assists in securing employment, and provides follow-up services.

Performance Responsibilities:

  • Attends all required meetings and trainings.
  • Works with students with significant disabilities to gain employment before high school exit.
  • Collaborates with HDI, OVR, and KDE to implement CWTP with fidelity.
  • Facilitates referral and application processes for VR services.
  • Works with students, parents, teachers, and employers on career interests and job possibilities.
  • Uses technical and computer applications effectively.
  • Prepares site-related documents and conducts vocational assessments.
  • Develops individualized employment plans and supports transition planning.
  • Maintains records and submits reports to OVR and HDI.
  • Coordinates transportation and promotes positive community relations.

Preferred Skills: Self-motivated, flexible, resourceful, computer skills, team player, experience with high school special education, working with businesses, and training individuals with disabilities.
