Equus Workforce Solutions in the U.S. seeks a FEP Case Manager to guide families through goal setting, barrier removal, and employment planning, using assessments to assign participants to appropriate W-2 service tracks.

You will collaborate with families to set employment goals, develop wraparound strategies such as job readiness and budgeting, coordinate with external providers, monitor progress, and maintain compliant documentation per program standards.

Formación

• Associate’s degree in human services, Social Work, or related field.
• Minimum of 2 years of experience in case management, employment services, or social services.
• Strong organizational and communication skills.
• Ability to work collaboratively with diverse populations and service providers.
• Proficiency in Microsoft Office and case management software.

Responsabilidades

• Conduct initial and ongoing assessments to determine participant needs, strengths, and barriers to employment.
• Assign participants to appropriate W-2 service tracks based on eligibility and assessment outcomes.
• Collaborate with families to establish employment and self-sufficiency goals, prioritizing barrier removal and skill development.
• Develop and implement wraparound strategies including job readiness, career planning, budgeting, and skills training.
• Coordinate activities outlined in the W-2 Individual Employment Plan (IEP) with those of external support providers.
• Monitor participant progress, update service plans, and adjust strategies as needed to ensure goal attainment.
• Maintain accurate and timely documentation in compliance with W-2 program standards.
• Connect families with community resources and services that support workforce participation.
• Ensure all interactions and documentation comply with applicable laws and program regulations.
