Job summary

African Community Services in Minneapolis seeks a dedicated case manager to deliver MFIP-based employment services to individuals facing barriers. You will conduct comprehensive needs assessments, provide intensive case management, and coordinate with other agencies to help participants meet program goals.

You will develop individual employment plans, document activities accurately, maintain participant files, and report outcomes to ensure compliance and progress toward self-sufficiency.

Qualifications

• Experience delivering MFIP services or similar
• Ability to conduct in-home visits and coordinate with partner agencies
• Strong documentation and reporting skills

This position involves intensive case management and employment services to individuals on MFIP who have multiple barriers preventing them from meeting program expectations.

Responsibilities Include:

• Complete a comprehensive needs assessment on all individuals referred to the program.


• Provide intensive case management, including home visits, meetings with other agencies, and advocacy for participants and their families.


• Develop individual employment plans and assist participants in accessing public and private resources to meet goals outlined in the plan.


• Complete all documentation and paperwork accurately and in a timely manner.


• Maintain participant files and submit reporting forms and other data as required by MFIP and ACS.


• Work closely with program staff to communicate participants' status, activities and their movement to compliance and self-sufficiency.


• Help participants develop and strengthen their self-esteem and work behaviors.


• Positively represent agency, program, and participant needs.
