Job summary

MyPath is seeking a Direct Support Professional in Waukesha, Wisconsin. You will provide direct care and supervision to individuals served, supporting health, safety, and well-being across daily routines.

Responsibilities include implementing ISPs/ITPs and BSPs, assisting with activities of daily living, documenting progress, and communicating with the care team to coordinate services. You will help with transportation and maintain a safe, respectful environment in compliance with policies.

Qualifications

• Provide direct care to and supervision of individuals served, ensuring their health, safety, and well-being.
• Implement ISPs/ITPs, BSPs, and program routines.
• Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
• Facilitate activities that promote skill development, independence, recreation, and community engagement.
• Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
• Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
• Safely transport individuals to appointments, activities, or community outings as required.
