Job summary

MyPath is seeking a Direct Support Professional in Waukesha, WI to provide direct care and supervision for individuals with special needs. You will implement ISPs/ITPs and BSPs, assist with activities of daily living, and support skill development and community engagement.

Responsibilities

• Provide direct care to and supervision of individuals served, ensuring their health, safety, and well-being.
• Implement Individual Service Plans (ISPs)/Treatment Plans (ITPs), Behavior Support Plans (BSPs), and program routines.
• Assist individuals with activities of daily living including personal care, meals, mobility, and household tasks.
• Facilitate activities that promote skill development, independence, recreation, and community engagement.
• Monitor and document progress, behaviors, health concerns, and incidents in accordance with policy.
• Communicate effectively with team members, supervisors, and other stakeholders to support coordinated care.
• Safely transport individuals to appointments, activities, or community outings as required.
• Maintain a clean, safe, and supportive environment while following all safety and regulatory requirements.
