Job summary

Apex Home Health, Hospice & Home Care is seeking a dedicated Physical Therapist Assistant (PTA) to provide skilled physical therapy under the supervision of a licensed Physical Therapist in home health settings.

The PTA will perform therapeutic exercises, gait training, and mobility training, then monitor progress, educate patients and caregivers, and document outcomes. Collaboration with the rehab team and adherence to HIPAA ensure safe, compassionate, patient-centered care.

Qualifications

• Graduate from an accredited PTA program.
• Current licensure or eligibility for licensure as a PTA.
• CPR/BLS certification or ability to obtain it.
• Knowledge of rehabilitation techniques and patient mobility.
• Strong communication, organizational, and interpersonal skills.
• Ability to work effectively within a multidisciplinary healthcare team.
• Commitment to delivering compassionate, patient-centered care.

Responsibilities

• Assisting with therapeutic exercises, gait training, balance activities, transfers, and mobility training.
• Monitoring patient progress and communicating changes to the supervising therapist.
• Educating patients and caregivers on home exercises, mobility techniques, and assistive device use.
• Completing accurate clinical documentation and maintaining a safe, organized treatment environment.
• Collaborating with the rehabilitation team to ensure continuity of care.
• Participating in meetings, quality initiatives, and continuing education.
• Ensuring compliance with HIPAA, safety standards, and organizational policies.
