Responsibilities

• Demonstrates ability to perform patient transfers with assistance from bed, chair or floor as needed.
• Designs an appropriate plan of care integrating goals, treatment, outcomes and discharge plans.
• Educates patients, families, caregivers, students and other health care providers using effective teaching methods.
• Evaluates clinical findings to determine PT diagnoses and outcomes of care.
• Documents in EMR and communicates with referring providers on patient progress and recommendations.
