Guardian Angel Home Care seeks a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area. You will provide high-quality, patient-centered care in patients' homes while collaborating with a supportive interdisciplinary team.

This role offers autonomy, flexible scheduling, and meaningful patient relationships. You will monitor conditions, administer treatments, document visits in HCHB, and educate patients and families on care plans.

Formación

• Active LPN license in good standing (Michigan).
• Graduate of an accredited practical nursing program.
• BLS/CPR certification current.
• Valid driver's license and reliable transportation.

Responsabilidades

• Provide skilled nursing care in patients' homes under RN/physician direction.
• Monitor patient condition, vital signs, and health status.
• Administer medications and treatments as ordered within LPN scope.
• Document visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to supervising RN/physician.
• Educate patients and families on medications, disease management, and care plan.
• Ensure compliance with federal, state, and agency regulations.
• Collaborate with interdisciplinary team to improve patient outcomes.
