Job summary

Guardian Angel Home Care Inc. is seeking a compassionate Licensed Practical Nurse (LPN) to join our home health team serving patients in the Grand Rapids area. You will provide patient-centered care in patients' homes and collaborate with a supportive interdisciplinary team.

This role offers autonomy with flexible scheduling and opportunities for professional growth. Ideal candidates are licensed in Michigan, with BLS/CPR certification and strong clinical and communication skills.

Qualifications

• Active Michigan LPN license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to work independently in a home health setting.

Responsibilities

• Provide skilled nursing care in patients' homes under the direction of RN and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Accurately document all visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and the plan of care.
• Ensure compliance with all federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
