Guardian Angel Home Care seeks a compassionate Licensed Practical Nurse (LPN) to join our home health team in the Grand Rapids area. You will provide patient-centered care in patients' homes while collaborating with an interdisciplinary team.

The role offers autonomy, flexible scheduling, and opportunities to build meaningful relationships with patients, with a focus on quality care and regulatory compliance.

Formación

• Active Michigan Licensed Practical Nurse (LPN) license in good standing.
• Graduate of an accredited practical nursing program.
• Current BLS/CPR Certification.

Responsabilidades

• Provide skilled nursing care in patients' homes under the direction of the Registered Nurse and physician.
• Monitor patient condition, vital signs, and overall health status.
• Administer medications and treatments as ordered and within the LPN scope of practice.
• Accurately document all visits and care provided in Homecare Homebase (HCHB).
• Communicate changes in patient condition to the supervising RN and physician.
• Educate patients and families on medications, disease management, and the plan of care.
• Ensure compliance with all federal, state, and agency regulations.
• Collaborate with the interdisciplinary team to achieve optimal patient outcomes.
