Job summary

Reliant Care Management Company in Massachusetts is seeking a compassionate LPN to support residents and care teams. You will contribute to assessments, documentation, and routine nursing duties in a resident-centered environment.

The role requires an active LPN license and a caring approach. You will work with a dedicated team, participate in admissions and discharges, coordinate care, and respond promptly to emergencies with a focus on patient comfort and safety.

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
