Job summary

Reliant Care Management Company seeks a compassionate LPN for a part-time role in Massachusetts. You will conduct assessments, admit and discharge patients, provide wound care, and supervise CNAs/CMTs per facility protocols.

Qualifications

• LPN licensure required.
• Part-time availability.
• Ability to supervise CNAs and CMTs.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
