Job summary

Qualifications

• LPN license issued by the State of Alabama.
• CPR certification required within 30 days of employment.
• One year of health care facility experience preferred.

Responsibilities

• Admit, transfer, and discharge residents as required.
• Monitor residents' physical and emotional status.
• Direct Nursing/Resident Care Assistants and report concerns to the Resident Care Coordinator.
• Document according to policies, procedures and regulations.
• Chart nurse notes to reflect care and resident responses.
• Report discrepancies and issues regarding resident care to the Resident Care Coordinator.
• Transcribe and/or check physician orders for execution.
• Administer medications per physician orders.
• Complete assessments on all assigned residents each shift.
• Report residents’ condition to oncoming staff and communicate with physicians and families.
• Provide direct care as needed according to the care plan and nursing standards.
• Perform other duties including special projects as assigned.
