Job summary

Adoray is seeking a licensed Certified Nursing Assistant (.8 FTE) to join our Baldwin, WI team. The CNA will perform routine patient care duties, assist with nursing tasks under RN supervision, and maintain accurate clinical records.

Prior home health or hospice experience is preferred and active Wisconsin registry status is required. The role emphasizes teamwork within an interdisciplinary group and adherence to the patient care plan, with attention to timely reporting of changes in condition

Qualifications

• Satisfactory completion of a CNA training program or competency test for Home Health Aide.
• Home health and/or hospice experience preferred.
• Current Driver's license.
• Name listed on Wisconsin registry as current and eligible to provide home health and hospice care.

Responsibilities

• Provide CNA-level patient care: bathing, skin/hair/nail care, transfers, ambulation assistance, toileting, medication self-administration, and light housekeeping.
• Assist with assigned nursing duties under RN supervision, such as dressing changes and catheter care.
• Notify supervisor of deviations from the activity plan and changes in patient condition within one hour of assessment.
• Complete clinical records related to duties and patient observations within 48 hours.
• Work as part of the interdisciplinary team and contribute to treatment plans.
