Responsibilities

• Provide medication reminders and support daily living activities.
• Assist with mobility and accompany clients to appointments or outings.
• Assist with personal care and hygiene as needed.
• Help with exercises and light physical therapy routines per plan.
• Run errands or accompany clients while shopping.
• Perform light housekeeping and meal preparation.
• Report incidents promptly to Care Coordinator and follow care plans.
