Qualifications

• Must have 1+ year of home care experience.
• Strong communication and interpersonal skills.
• Ability to follow care plans and safety guidelines.

Responsibilities

• Provide medication reminders and monitor client condition.
• Assist with ambulation, mobility, and transportation to appointments.
• Assist with personal care and hygiene.
• Support client with exercises and physical therapy as directed.
• Run errands or accompany clients shopping as needed.
• Perform light housekeeping and meal preparation.
• Report incidents to Care Coordinator and respond to emergencies.
• Follow the established care plan and maintain client safety.
