Job summary

VNS Health seeks a Peer Specialist/Advocate to engage with clients in a Behavioral Health context across home, clinic, and community settings. You will use lived experience to build trust, educate on services, and support recovery goals as part of an interdisciplinary team.

You will conduct outreach, assist with care planning, document in EMR, and help clients access benefits and community resources. Bilingual English/Spanish preferred; HS diploma required.

Qualifications

• High school diploma or equivalent required.
• Experience in mental health, substance use treatment, health care or human services preferred.
• Strong communication and relationship-building skills.
• Ability to work independently and in an interdisciplinary team.
• Bilingual English/Spanish preferred.

Responsibilities

• Conducts outreach to locate and enroll clients into programs and services.
• Takes part in an interdisciplinary team to discuss care needs and plan goals.
• Educates clients about program services, benefits and self-help techniques.
• Documents in EMR according to policy and regulatory requirements.
• Provides navigation to connect clients with community-based supports.
