Qualifications

• This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice.
• The nurse is accountable for the quality of nursing services delivered by self or others under their direction.
• Utilizes specialized knowledge, judgment, and nursing skills to assess data and plan, provide and evaluate care.

Responsibilities

• Assesses patient needs across physical, psychological, social, spiritual, educational and discharge planning domains.
• Formulates a plan of care with realistic, measurable goals in collaboration with patient/family.
• Implements plan of care, coordinates and supervises activities of the health care team.
• Evaluates patient response to care and revises the plan as needed.
• Documents assessment, planning, implementation and evaluation in the patient record.
