Penn Medicine is seeking a Mental Health Intake Coordinator to support the Crisis Response Center at the Hospital of the University of Pennsylvania Cedar campus. You will conduct emergency psychosocial assessments, coordinate care plans, and assist with admissions and referrals.

Under Nurse Manager supervision, you will work 36 hours per week (0.9 FTE) on day shift, collaborating with a multidisciplinary team to deliver compassionate care and crisis intervention.

Qualificações

• H.S. Diploma or GED required.
• Equivalent experience: 2+ years in sociology, social welfare, psychology, gerontology or other social science and 2 years in human services with 1 year mental health direct care.
• Bachelor’s degree preferred (BA/BS).
• Education specialization in Social Work, Psychology, MFT, Counseling or healthcare field.

Responsabilidades

• Provides emergency psychosocial evaluations and services to patients and families in the Crisis Response Center.
• Performs patient care activities including activities of daily living, vital signs, and meal/bathing assistance.
• Collaborates with Psychiatric Crisis Service professionals and documents care in progress notes.
• Arranges inpatient admissions, outpatient referrals, and coordinates transportation or transfers to other facilities.

Formação académica

H.S. Diploma/GED

Equivalent Experience: 2+ years sociology/psychology etc. + 2 years human services with 1 year mental health direct care

Bachelor of Arts or Science preferred

Education Specialization: Social Work, Psychology, Marriage and Family Therapy, Counseling or any Healthcare related field
