Job summary

LiveWell Homecare Agency is seeking a licensed Pharmacist to lead safe, evidence-based medication use within our care teams. The Pharmacist will verify orders, educate patients and families, and collaborate with clinicians to optimize therapy and outcomes.

Responsibilities include ensuring medication safety, participating in quality initiatives, supervising pharmacy technicians, and precepting students. A strong clinical background and commitment to ongoing professional development are essential.

Qualifications

• Pharmacy degree (PharmD or PharmB) and active pharmacist license.
• Minimum 2 years of pharmacy experience including technician, intern, or pharmacist roles.
• Strong clinical knowledge and effective communication and teamwork.
• Commitment to ongoing professional development.

Responsibilities

• Deliver clinical pharmaceutical care, including medication safety and adherence to standards (TJC, BOP, CMS).
• Verify medication orders for accuracy and appropriateness.
• Educate patients, families, and healthcare team members on medication use.
• Participate in quality and safety initiatives, such as adverse event reporting, clinical guideline development, and medication use evaluation.
• Ensure efficient, safe medication distribution, collaborating with pharmacy staff, and overseeing pharmacy technicians.
• Support professional development by precepting students and participating in educational activities.
• Contribute to departmental programs, services, and practice improvements.
• Maintain compliance with organizational policies and standards.
