The UW Medical Center - Montlake Social Work Department in Seattle has an outstanding opportunity for a Social Worker within the Emergency Department. Per Diem, rotating/variable shift.

Join a collaborative team that provides psychosocial assessment, discharge planning, crisis intervention, and resource referrals to support patients’ medical conditions and transitions across care settings.

Qualifications

• Master of Social Work from a program accredited by the Council on Social Work Education.
• At least one year of full-time social work experience in a health care setting or equivalent.
• Credentialing within 90 days of hire per Washington state requirements.

Responsibilities

• Provide psychosocial support to patients and families.
• Develop and implement safe and timely discharge plans.
• Coordinate care across the multidisciplinary team.
• Provide case management and counseling in specialized programs.
• Make referrals to hospital and community agencies as needed.
