Job summary

Qualifications

Masters degree in Social Work and at least one year of healthcare experience.
Licensed Social Worker or Licensed Clinical Social Worker.
Basic Life Support certification.

Responsibilities

Responding to referrals and case finding for high-need populations such as those with new diagnoses, terminal illnesses, homelessness, or complex social issues.
Conducting psychosocial assessments, developing and implementing care plans, and documenting interactions according to standards and regulations.
Assisting patients and support systems with coping strategies, discharge planning, and connecting them to community resources.
Providing consultations on abuse, neglect, advance care planning, and legal decision-making.
Participating in performance improvement initiatives, departmental activities, and staff development.
