Job summary

MercyOne Medical Group in Des Moines, IA is seeking a Clinical Pharmacist to optimize pharmacotherapy and participate in interdisciplinary rounds. The role covers designated areas with rotating day/evening shifts and every third weekend. Responsibilities include MTMs, emergencies participation, and precepting students.

Ideal candidates hold a BS or Pharm.D. from an ACPE-approved college, and ASHP PGY-1 residency or 3 years hospital experience; PGY-2 or 3 years in area preferred.

Qualifications

• Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy.
• ASHP-accredited PGY-1 Pharmacy Practice Residency or 3 years hospital experience required.
• PGY-2 residency or 3 years experience in the relevant clinical practice area preferred.
• BLS certification within 3 months of hire.
• ACLS and/or PALS certification within 3 months of hire based on practice area(s). Licensed, or eligible for licensure, pharmacist in Iowa. Must be licensed within 3 months of hire.
• BCPS or similar board certification within 2 years of hire or 10 years of experience in the relevant area.
• Other certifications if requested by supervisors/managers.
• Proof of completion of Mandatory Reporter abuse training within 3 months of hire.

Responsibilities

• Assess patient data to identify medication-related problems through profile reviews, pharmacy consults, and rounding.
• Participate in medical emergencies and formal alerts such as trauma, stroke, sepsis, and STEMI as required.
• Review providers' orders for appropriateness based on patient age, weight, and comorbidities.
• Consult with providers and nurses to optimize pharmacotherapy across the continuum of care.
• Provide timely drug information responses and ensure policy compliance.
• Serve as preceptor for pharmacy students/residents and assist with training.
• Assist nursing with medication administration per policy and assignment.
• Document patient care activities in the EMR per guidelines.
• Maintain quality and safety standards, report errors, and drive process improvements.
