The ideal candidate holds a BS in Pharmacy (PharmD preferred), a current state pharmacist license, and exposure to hospital operations.

Qualifications

• Current state Registered Pharmacist License.
• B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
• Completion of PGY-1 or >3 years qualifying experience required.
• Postgraduate training PGY-2 preferred.
• Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
• BLS Required.
• ACLS Preferred.
• Experience in hospital pharmacy operations preferred.

Responsibilities

• Coordinates activities of the pharmacy department to provide high quality pharmaceutical care to all patients.
• Conducts 50% or more of professional time in direct patient care activities including rounding, counseling, or participation in patient care committees.
• Involves direct observation and evaluation of patient and medication-related needs; initiates or adjusts pharmacotherapy; monitors patients in collaboration with health professionals.
