Qualifications

• Current state Registered Pharmacist License.
• B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
• Completion of PGY-1 training or 3+ years of qualifying hospital experience.
• PGY-2 training preferred.
• Board certification in a BPS clinical specialty preferred.
• BLS Required; ACLS preferred.
• Experience in hospital pharmacy operations preferred.

Responsibilities

• Coordinate activities of the pharmacy department to provide the highest quality of pharmaceutical care.
• Direct patient care includes rounding, counseling, and participation in patient care committees.
• Initiate, modify, or discontinue patient-specific pharmacotherapy and monitor therapy in collaboration with health professionals.
• Care scope may include General Medicine/Surgical, ICU, OB-GYN, Pediatrics, ER, Oncology, Peri-operative, Geriatrics and Outpatient Services.
