Job summary

The Care Navigator position at Partners in Care Foundation will support Kaiser Permanente Medi-Cal members and dually eligible Medicare/Medi-Cal members in real time within the Emergency Department, focusing on high-touch engagement, linkage to services, and care coordination.

This role collaborates with KP Sacramento ED leadership, care coordination teams, social work, TCS, CCM, Medicare programs, and community partners to connect members to ECM, TCS, MFA, and other supports, aiming to improve

Qualifications

• Experience with Medi-Cal, Medicare, or CalAIM programs.
• Familiarity with Emergency Department operations.
• Strong communication and patient engagement skills.
• Ability to navigate complex systems and coordinate across multidisciplinary teams.

Responsibilities

• Identify eligible members for outreach from real-time ED census lists and engage in-person in the ED.
• Support enrollment into Enhanced Care Management (ECM) and coordinate with authorization teams when needed.
• Connect members to Kaiser Permanente services (ECM, TCS, CCM, MFA) and community resources.
• Conduct post-visit follow-up calls and document interventions per pilot workflows.
