Job summary

Partners in Care Foundation is seeking a Care Navigator embedded in the Kaiser Medical Center Emergency Department to support Medi-Cal and dual eligible members. The role emphasizes high-touch engagement, real-time navigation, and linkage to Kaiser Permanente services, CalAIM programs, and community resources.

You will collaborate with ED leadership, care coordination teams, social work, and Medicare teams to improve care coordination, reduce avoidable ED visits, and smooth transitions after

Qualifications

• Experience with Medi-Cal, Medicare, dual-eligible populations, or CalAIM programs.
• Familiarity with hospital or Emergency Department operations.
• Strong communication and patient engagement skills.
• Ability to navigate complex systems and coordinate across multidisciplinary teams.
• Experience with care coordination or social service linkage preferred.
• Adaptable to fast-paced pilot workflows.

Responsibilities

• Review real time ED census lists and identify eligible members for outreach.
• Conduct in person member engagement within the Emergency Department.
• Coordinate enrollment into ECM and liaise with authorization teams.
• Provide navigation for members enrolled in ECM or other care coordination programs.
• Connect members to Kaiser Permanente services including: Transitional Care Services (TCS), Complex Case Management (CCM), Social Work, Medical Financial Assistance (MFA).
• Document outreach and interventions per pilot workflows.
