Job summary

GBMC HealthCare is seeking a discharge planner in the Emergency Department Behavioral Health unit to support multidisciplinary treatment teams. The role focuses on assessing patient needs, coordinating eligible home or facility placements, and facilitating communication with families and care providers.

The position requires a bachelor’s degree (or progress toward one within 3 years) and 3+ years of psychiatric/health care discharge planning experience.

Qualifications

• Bachelor's degree from an accredited institution required; OR must be working towards a bachelor’s degree and completed within 3 years of hire.
• Three years of progressively responsible work experience in psychiatric/chemical dependency field or related health care setting.
• Knowledge of available community resources.
• Advanced skills in oral and written communication.
• Skill in organizing workload and balancing many tasks.
• Skills in time management.
• Ability to evaluate data gathered for medical plan of treatment.
• Ability to work with a multidisciplinary team in a medical setting.

Responsibilities

• Gathers and evaluates information to prepare for patient discharge.
• Meets with patients and families in the Emergency Department to obtain psycho-social history for discharge planning.
• Contacts ED personnel and obtains medical records to determine discharge needs.
• Communicates with care team to exchange information on social, emotional or environmental problems.
• Collects advance directives, MOLSTs, custody agreements, psychiatric certifications and other court orders relevant to discharge planning.
• Educates patients and involved parties about the discharge process during psychiatric evaluation.
• Obtains consent signatures for documentation by patient, parent or guardian if pediatric.
• Provides bed availability or disposition updates to patients or guardians.
• Maintains and updates electronic medical records and ensures HIPAA compliance.
• Collaborates with crisis bed and Child Protection Program liaisons as needed.
• Assesses home care needs and placement options; arranges or secures psychiatric facility placements when recommended.
• Submits insurance pre-authorization for placement.
• Keeps the treatment team informed of discharge plan, status, barriers, and transport ETA.
• Discusses discharge plan with parent/guardian for pediatric patients.
• Organizes discharge packets for transfers to inpatient psychiatric facilities.
