Qualifications

• Bachelor's degree required or must be completed within 3 years of hire.
• Three years' progressively responsible work experience in psychiatric/chemical dependency field or related health care setting.
• Knowledge of available community resources; advanced skills in oral and written communication.
• Ability to organize workload and balance many tasks; strong time management.
• Ability to evaluate data gathered for medical plan of treatment.
• Ability to work with a multidisciplinary team in a medical setting.

Responsibilities

• Gathers and evaluates information to prepare for patient discharge from the ED.
• Meet with patients and families to obtain psycho-social history relevant to discharge planning.
• Coordinate with ED staff and obtain medical records to determine discharge needs.
• Communicate with treatment team about social, emotional, or environmental issues.
• Collect and manage advance directives, MOLST, custody agreements, and other documents.
• Educate patients and families about the discharge process and obtain necessary consent signatures.
