Job summary

myPlace Health in Los Angeles County is seeking a Medicaid Specialist to help underserved frail seniors apply for Medi-Cal or Medicare, enabling them to stay at home as long as possible. You will manage Medi-Cal applications, conduct home visits, and support enrollment outreach.

The role requires knowledge of the California Medi-Cal system, 2–4 years of relevant experience, and collaboration with state/county eligibility workers. Bilingual ability is preferred.

Qualifications

• Knowledge of California Medi-Cal system and Social Security process.
• 2–4 years of relevant experience.
• Experience with state/county eligibility workers.
• Experience with frail elderly.
• Bilingual in Spanish, Cantonese, Mandarin, or Vietnamese preferred.

Responsibilities

• Initiate and manage Medi-Cal applications for participants.
• Conduct home visits to complete Medi-Cal applications.
• Assist participants, families, and caregivers with Medicaid re-determination or new applications.
• Maintain enrollment documentation in CRM and ensure compliance.
• Adhere to DHCS and CMS regulations for PACE marketing practices.
• Collaborate with Finance to prepare reports for data analysis.
• Validate Medicaid payments in support of Finance.
• Monitor and renew participants' Medicaid eligibility (recertification).
• Support the Enrollment and Outreach Specialist as needed.
• Provide community resources to participants or families and caregivers.
• Coordinate with county/state specialists to submit required documentation.

The role involves managing Medi-Cal applications, conducting home visits, collaborating with finance, and supporting enrollment outreach.

Responsibilities

• Initiate, assist, and manage Medi-Cal applications for prospective participants, ensuring timely and accurate completion.
• Conduct home visits to complete the Medi-Cal application process.
• Assist participants, families, and caregivers in completing all necessary paperwork for Medicaid re‑determination or new applications.
• Maintain documentation and record all enrollments in CRM software, ensuring accuracy and compliance.
• Strictly adhere to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program.
• Collaborate with Finance Department to prepare various reports for data analysis purposes.
• Validate Medicaid payments in support of the Finance department.
• Monitor and maintain participants’ Medicaid eligibility (recertification of Medi-Cal).
• Support the Enrollment and Outreach Specialist as needed.
• Provide community resources to prospective participants or families and caregivers.
• Collaborate with or contact county/state government specialists to ensure submission of required documentation or to obtain additional.
• All other duties as assigned.

Requirements

• Proof of tuberculosis screening within the last year (or provide a negative screening certificate) is required as a condition of employment.
