myPlace Health, a PACE program in Los Angeles County, seeks a Medicaid Specialist to help frail seniors apply for Medi-Cal and Medicare. This role focuses on keeping participants at home and ensuring timely, accurate processing.

The ideal candidate will have California Medi-Cal experience, 2-4 years in eligibility work, and bilingual abilities. Onsite work with a comprehensive benefits package is provided.

Qualifications

• Knowledge of California Medi-Cal system and Social Security processes.
• 2-4 years of relevant experience with State/County eligibilities workers.
• Bilingual: Spanish, Cantonese, Mandarin, Vietnamese preferred.

Responsibilities

• Initiates, assists, and manages Medi-Cal applications for prospective participants.
• Conducts home visits to complete the Medi-Cal application process and renewals.
• Maintains documentation and enrollment in CRM with accuracy and compliance.
• Complies with DHCS and CMS regulations regarding PACE marketing practices.
• Collaborates with Finance to prepare reports and validate Medicaid payments.

This person will have an opportunity to help bring the PACE (Program of All-Inclusive Care for the Elderly) to many underserved frail seniors in the community. This individual will help develop the process of helping frail seniors to apply for Medicaid or Medicare.This Medicaid Specialist is a key player in keeping the underserved frail seniors continue to live at home as long as they wish. This ideal candidate brings direct experience understanding the Medicaid and Medicare new/renew process.RESPONSIBILITIES:Initiates, assists, and manages Medi-Cal application for prospective participants, ensuring timely and accurate completion of the process.Conduct home visit to complete the Medi-Cal application process. Assist participants, families/caregivers in completing all necessary paperwork needed for Medicaid re-determination or new applications.Maintain all documentations and record all enrollment in CRM software and ensure accuracy and complianceStrictly adhering to DHCS and CMS regulations and policies regarding marketing practices for the PACE ProgramCollaborate with Finance Department to prepare variety of reports for data analysis purpose. Supports the Finance department to validate Medicaid payment.Monitor and maintain participant’s Medicaid eligibility (recertification of Medi-Cal)Support the Enrollment and Outreach Specialist as needed.
