Job summary

myPlace Health in Los Angeles County is seeking a Medicaid Specialist to initiate and manage Medi-Cal applications for frail seniors, including home visits and document handling. You will ensure timely processing, maintain CRM enrollment records, and comply with DHCS and CMS policies.

The ideal candidate has 2–4 years of relevant experience, knowledge of Medi-Cal and Social Security processes, and bilingual abilities. This onsite role offers income security and a comprehensive benefits package.

• Experience with frail elderly population.

Responsibilities

• Initiates, assists, and manages Medi-Cal application for prospective participants, ensuring timely and accurate completion of the process.
• Conduct home visit to complete the Medi-Cal application process. Assist participants, families/caregivers in completing all necessary paperwork needed for Medicaid re-determination or new applications.
• Maintain all documentations and record all enrollment in CRM software and ensure accuracy and compliance
• Strictly adhering to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program
• Collaborate with Finance Department to prepare variety of reports for data analysis purpose. Supports the Finance department to validate Medicaid payment.
• Monitor and maintain participant s Medicaid eligibility (recertification of Medi-Cal)
• Support the Enrollment and Outreach Specialist as needed. Provide community resources to prospective participants or families/caregivers
• Collaborate with or contact county/state government specialist to ensure the submission of required documentation or to obtain additional.

About this Role

This is a great opportunity for an individual who is passionate about serving the underserved community in Los Angeles County. This person will have an opportunity to help bring the PACE (Program of All-Inclusive Care for the Elderly) to many underserved frail seniors in the community. This individual will help develop the process of helping frail seniors to apply for Medicaid or Medicare.

This Medicaid Specialist is a key player in keeping the underserved frail seniors continue to live at home as long as they wish. This ideal candidate brings direct experience understanding the Medicaid and Medicare new/renew process.

QUALIFICATIONS & EXPERIENCE:

• Knowledge of California Medi-Cal System and Social Security process
• 2-4 years of relevant exp
• Experience working with State/County eligibilities workers
• Experiences working with Frail Elderly
• Bilingual: Spanish, Cantonese, Mandarin, Vietnamese preferred
