Job summary

myPlace Health in Los Angeles County is seeking an Eligibility Coordinator to help frail seniors access Medi-Cal and Medicare benefits through the PACE program. The role focuses on initiating Medi-Cal applications, conducting home visits, and guiding participants and families through the enrollment process.

You will maintain documentation in our CRM, ensure compliance with DHCS and CMS rules, and collaborate with Finance to produce data reports.

Qualifications

• Knowledge of California Medi-Cal and Social Security processes.
• 2–4 years of relevant experience.
• Experience with state/county eligibility workers.
• Experience with frail elderly populations.
• Bilingual: Spanish, Cantonese, Mandarin or Vietnamese preferred.

Responsibilities

This person will have an opportunity to help bring the PACE (Program of All-Inclusive Care for the Elderly) to many underserved frail seniors in the community. This individual will help develop the process of helping frail seniors to apply for Medicaid or Medicare.This Medicaid Specialist is a key player in keeping the underserved frail seniors continue to live at home as long as they wish. This ideal candidate brings direct experience understanding the Medicaid and Medicare new/renew process.**RESPONSIBILITIES:*** Initiates, assists, and manages Medi-Cal application for prospective participants, ensuring timely and accurate completion of the process.* Conduct home visit to complete the Medi-Cal application process. Assist participants, families/caregivers in completing all necessary paperwork needed for Medicaid re-determination or new applications.* Maintain all documentations and record all enrollment in CRM software and ensure accuracy and compliance* Strictly adhering to DHCS and CMS regulations and policies regarding marketing practices for the PACE Program* Collaborate with Finance Department to prepare variety of reports for data analysis purpose. Supports the Finance department to validate Medicaid payment.* Monitor and maintain participant’s Medicaid eligibility (recertification of Medi-Cal)* Support the Enrollment and Outreach Specialist as needed.
