Job summary

Weber School District in Ogden, Utah is seeking a Student Services District Facilitator to support elementary schools (K-6) under the direction of the Student Services Director. The role focuses on programming, training and implementing support for students within MTSS and district guidance services.

Responsibilities include collaboration with counselors, teachers and families, data collection, development of intervention and reintegration plans, and participation in professional learning

Qualifications

• Hold a current Utah Educator License - School Counselor and CCR credential
• Master degree preferred.

Responsibilities

• Work with counselors, faculties and administrators supporting students
• Advocate on the student’s behalf
• Implement and/or support MTSS-based prevention/intervention strategies
• Collect data and record information to evaluate intervention effectiveness
• Attend student staffing meetings regularly
• Collaborate with families to write intervention and support plans
• Support elementary school counselors in counseling and guidance services
• Participate in data projects and annual reports
• Develop reintegration plans for students returning from treatment
• Coordinate enrollment of youth-in-care students
• Participate in professional learning communities
• Maintain confidential records as required by law
• Communicate effectively with teams and parents
• Drive to school sites in districts
• Perform other related duties as assigned
