Qualifications

  • Minimum of two years’ experience in human services or case management.
  • Knowledge of local social service agencies focusing on low-income/at-risk elders.
  • Ability to work effectively with diverse populations.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Conduct outreach to inform residents about the program and services.
  • Recruit and enroll program participants.
  • Develop individual service plans with measurable goals.
  • Coordinate referrals and provide onsite support for participants.
  • Meet clients in person, by phone, and in writing; monitor progress.
  • Build and maintain a network of community resources.
  • Create and maintain a directory of service resources.
  • Arrange education sessions on tenant rights, elder issues, and aging-in-place.
