Qualifications

• Participates in Hospital Elder Life Program activities.

• May perform additional related duties as needed.

Responsibilities

• Evaluate older inpatients at high risk and enroll them in HELP.
• Train and assign volunteers for patient interventions.

• Support geriatric care plans and interdisciplinary care.
• Document patient data and monitor program outcomes.
