Job summary

Center for Elders’ Independence is a PACE organization providing integrated health care for the elderly. The Chaplain role reports to the Director of Behavioral Health and coordinates with the IDT to deliver spiritual/pastoral care across the CEI continuum.

Responsibilities include providing spiritual support, end-of-life counseling, interfaith services, and education for the care team. Requires a theology-related Master’s degree, CPE, and ecclesiastical endorsement, with bilingual skills

Qualifications

• Master’s Degree in Theology, Divinity, Spirituality, or Pastoral Ministry is required.
• 4 units of Clinical Pastoral Education (CPE) are required.
• Board Certification Chaplain is preferred; the text indicates board certification through NACC or NAJC is required.
• Two years of healthcare, palliative care, hospice, or hospital chaplaincy experience is preferred.
• An ecclesiastical endorsement from a recognized denomination is required.
• Proficiency with multiple theological perspectives; strong written and verbal communication skills.
• Experience with EMR systems is advantageous.
• Bilingual preferred (Mandarin, Cantonese, Spanish).

Responsibilities

• Provide spiritual/pastoral support to participants and families across the CEI continuum of care.
• Support Palliative Care and Hospice goals and outcomes.
• Prepare reports on pastoral encounters and referrals.
• Counsel participants and families in advance care planning and end-of-life decisions.
• Coordinate with other spiritual care providers involved in the participant’s care.
• Lead interfaith worship, prayer services, memorials, and group activities.
• Educate teams on spiritual needs of older adults and serve as a resource on spirituality.
• Offer discussions on beliefs and values across diverse philosophies.
• Document counseling in the medical record.
• Collaborate with other professionals to resolve spiritual/ethical issues.
• Work with the IDT to evaluate care responses.
• Collaborate with community religious representatives to ensure continuity of spiritual care.
• Act as a resource for bereavement support.
• Participate in Behavior Health and IDT meetings to review care plans.
• Support planning to integrate spiritual care across centers.
• Lead spiritual groups and presentations on spiritual care services.
• Communicate with participants and families about spiritual care options.
• Develop and evaluate spiritual care services for outcomes and quality improvements.
• Attend in-services and staff meetings.
